I had this combination on a Falcon 030 back in 2010, but despite eventually stumbling across the correct software (MROS / HDDRIVER etc) and driver combination to make it work, I never did figure it out.  I got the last version of Cubase Audio working on the Falcon 030, and very impressive it was too.  Shame as I probably would have looked for a second CBX-D5 for the full 8 tracks.
