Great book and I bought mine back in 1996 and still have it.  Quite large page size, so not A4 flatbed scanner friendly, and I was going to upload it online.  I tried using a camera pointing down on a tripod, but the results were not good enough.

Just realised its already on archive.org  .  Seems to be two PDF versions, but not sure what the differences are yet.
