ACCORDING to my brief research
it appears this connector dissappeared sometime after 2005

the actual connector parts were referred to as DA-15 or DB-15 dependong on male or female,
it was often found also as an 8x8 pin block header on the motherboard to be able to add a ribbon cable connection to and pci bracket module to be able to access it with midi or joystick hardware
